- Pleading or other documents for docketed cases (4 copies each; at least 2 original copies of pleadings)
- Pleading or other documents for complaints with reference number (1 original copy or as indicated in the Complaint Checklist Form [OMB Form 6])
- When applicable, proof of service (e.g. registry receipt [RR], RR number, or affidavit of personal service) (1 copy)
(1 photocopy of the front and back of the ID to be compared with the original)
